This notice describes Premier Acupuncture of Boulder’s policy on how medical information about you may be disclosed, how you can access this information, and how your privacy is being protected. In order to maintain the level of service that you expect from this office, it may be necessary to share limited personal and medical information with your insurance company, with Worker's Compensation (and your employer should this be the case), or with other medical practitioners that you authorize.

Safeguards at My Clinic & What They Include:

     •Limited access to facilities where information is stored

     •Requirements for third parties to contractually comply with privacy laws

     •All medical history files, records, & consent forms are kept on permanent file

     •All SOAP notes & treatment notes are kept on encrypted digital files

Information I Gather & Use in administering your health care may include the following, non-public personal information:

     •About financial transactions with me (billing)

     •From your medical history, treatment notes, all test results, and any letters, faxes, emails, or telephone conversations to or from other health care     providers

    •From health care providers, insurance companies, workman’s comp and your employer, and other third party administrators
